1. 1. Completely Filled application form with attested photograph pasted on the space provided. Application form should be attested by Nursing Superintendent ( if Nursing Superintendent not available then attested by the head of department i.e. M.S/ MD/ MOI)

• Photograph taken on High class Kodak Paper
• Frontal view
• White Background
• Hair tied at the back
• One photograph attested by Nursing Superintendent ( If Nursing Superintendent Not available then attested by the head of department i.e. M.S/ MD/ MOI)
• Two photographs without attestation

  Note: Candidates are instructed to send three same photograph to Delhi Nursing Council, Ahilya Bai College of Nursing Building, Lok Nayak Hospital, New Delhi-110002 with DNC Registration Receipt Copy by post.

2. Scanned (duly attested) of 10th class Mark sheet.
3. Scanned (duly attested) of 10th class certificate
4. Scanned (duly attested) of 12th class Mark sheet
5. Scanned (duly attested) of 12th class certificate
6. Scanned (duly attested) of mark sheets of all three / four years examination Issued by Council/Examination Board/Universities.
7. Scanned (duly attested) of diploma/ degree certificate issued by Examination Board / Nursing Council / University.
8. Address proof of the nursing training institute with complete address from where the training is obtained any one document listed below:-
• School or college I-card.
• Character certificate
• Course completion certificate
• Transcript
• Fee receipt
9. Proof of being employed in Delhi
• identity card
• letter of appointment/ letter of selection (offer letter)
• recent salary slip.
10. Scanned copy of your AADHAAR CARD.
11. Scanned Parent Council Registration Certificate (Duly attested) (Front and back) (RN & RM). Valid renewal Registration Certificate.
12. Scanned Registration certificate (duly attested)of other councils(if re-registration done in other state) Valid renewal Registration Certificate.
13. Submission of original Registration certificate for Rajasthan, Kerala, Andhra Pradesh and Telangana.
