SEQUENCE OF DOCUMENTS TO BE ATTACHED FOR RE-REGISTRATION

1. Upload completely filled application form with photograph (self-attested)
   Photographs Specification:

   • Frontal view
   • White Background
   • Hair tied at the back
    

2. Upload the following original documents:
   

   • 10th class certificate
   • 12th class certificate
   • Mark-sheets of all three / four years or cumulative issued by Council/Examination Board/Universities.

3. Diploma/ degree certificate issued by Examination Board / Nursing Council / University.

4. Address proof of the nursing training institute with complete address from where the training is obtained any one document listed below:-
   

   • School or college I-card.
   • Character certificate
   • Course completion certificate
   • Transcript

5. AADHAAR CARD

6. Parent Council Registration Certificate (Front and back) (RN & RM) Valid renewal Registration Certificate.

7. Registration certificate of other councils (if re-registration done in other state) Valid renewal Registration Certificate.

8. Submission of original Registration certificate for Punjab, Rajasthan, Kerala, Andhra Pradesh and Telangana as required by these Councils.

9. Submission of Punjab Nurses Registration Council original Registration certificate along with the Challan (Candidate need to visit PNRC website www.pnrconline.in to generate challan and then deposit their required fee of PNRC for NOC in any branch of Axis Bank vide letter NO:-PNRC: -2019/3362 Dated: - 13.5.19).
